Stormont Vail Health, Shawnee County Health Department and Heartland Healthy Neighborhoods Request Public Feedback on the Shawnee County Community Health Needs Assessment

Improving the health of the community is the number one priority for health professionals in Shawnee County. To better address the needs of the community, Stormont Vail and the Shawnee County Health Department are collaborating to perform a Community Health Needs Assessment (CHNA) for the residents of Shawnee County, with the goal to understand the scope of wellness in the community and the challenges in reaching optimal health. 

Public input is important to understanding the needs in the community. The public is invited to provide feedback through a survey.

The survey, available in both English and Spanish, asks questions about health needs for individuals, families and the community. The survey should take approximately 10 minutes to complete and all responses are confidential.

A paper copy of the survey can be requested by calling Karla Hedquist, director, Community Health Engagement, at (785) 270-0139. 

The survey will close on February 28, 2021. The feedback received will be combined with data to create a comprehensive voice of the community at a Shawnee County town hall held in April.
