Midland Care Expands Integrated Care Model With Acquisition of Vintage Medical Associates

Midland Care Connection has announced that its subsidiary, Midland Care Physician Services, has acquired Vintage Medical Associates (VMA), a primary care practice focused on serving older adults across northeast Kansas. This acquisition allows Midland Care to more closely coordinate medical and support services, enable streamlined care transitions, and improve communication among providers.

Vintage Medical Associates provides primary care and medical director services within senior housing environments, including rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. Dr. Timothy Heston and VMA providers currently serve more than 600 patients across 33 facilities in Topeka, Manhattan, and Lawrence.

As part of the acquisition, all VMA medical providers and office staff will join Midland Care Physician Services. Midland Care first partnered with Dr. Heston and the VMA team in 2023 through its Accountable Care Organization, Responsive Care Solutions.

The addition of primary care services enhances Midland Care’s ability to align clinical care, improve outcomes, and support patients throughout their healthcare journey. This integrated care model allows patients and families to coordinate services through a single organization, making it easier to manage chronic conditions, arrange follow-up care, and access resources.
